The deadline to submit either form is April 10th
In an ideal world, we would use up, give away, or destroy all of our hametz before the start of Pesah. The rabbis, however, recognized that not everyone had this capability and instituted a legal remedy. Through mekhirat hametz - the sale of hametz to a non-Jew - we arrange a technicality that removes the hametz from our possession. By filling out either the online or physical form linked above, you authorize Rabbi Rein to "sell" all hametz so that they are not accidentally consumed during the holiday.
Although in other sales, the seller receives money, in this one, it is customary for the seller to include with the sale authorization a donation, called ma’ot hittin, “money for wheat,” which will be placed in the Rabbi’s Discretionary fund to help those in need.

The Rabbinical Assembly's Guide for Pesah 5784
The last several years have had us experiencing Passover in non-traditional formats. To help us navigate this uncertain terrain, the Committee on Jewish Law and Standards (CJLS) provided guidelines for the moment we found ourselves. This year, as we move out of the Pandemic and towards a new normal, the CJLS has once again provided updated Pesah guidelines with information that includes kashering, destruction of hametz, food purchases and more.
